WHY TRANSITION?
Students with disabilities:
Need a better chance to succeed in adult life
Don’t receive needed post-school supports
Are not generally as successful as adults in the general population
The concept of transition is simple:
Help students and families think about life after high school and learn about available resources
Identify long-range goals.
Design high school experiences for success after high school.
Empower students to accomplish outcomes based on choices.
Help students access services before they leave high school.
To enhance transition services delivered to students in the Denver Public Schools, the ITT’s purpose is to:
Develop a strong, interagency transition planning process.
Collaborate and coordinate service delivery among agencies to support transition.
Develop a support for transition in the Denver community.
Identify barriers and gaps in services for students in transition
